Please remember to bring your insurance card and picture ID.

The office staff of USC will attempt to call you regarding any deductible and/or co-pay that will need to be paid prior to your surgery/procedure. In most cases we will be able to give you an estimate of the cost of your surgery/procedure.
When you arrive, our office staff will help you finalize your financial arrangements before your surgery/procedure.

Whatever type of payment arrangement is decided upon (policy dictates that these arrangements are made BEFORE your surgery/procedure), the amount of your co-pay and/or deductible due must be paid prior to your surgery /procedure.

Please understand that your co-pay and deductible are agreed upon between yourself and your insurance plan. As members of your provider network, we are obligated to collect the co-pay and/or deductible designated by them or face the possibility of being removed from their network. You should contact your insurance plan for more information.

Your surgery/procedure will produce a billing for several providers. They include, but are not limited to,

​Surgeon
Facility (USC)
Anesthesia
Pathology (if specimen is taken)
Radiologist (if X-ray is used) 

If you have any questions regarding your coverage for any of these charges, your best resource is your insurance company. We verify your insurance coverage and will be happy to give you the information that they have given us, however the insurance plan has the final say in any coverage question.  
We accept cash, check, money order, Discover Card, American Express, VISA and MasterCard for any required co-pay or deductible.

PATIENT RIGHTS UNDER FLORIDA TRANSPARENCY ACT OF 2016

Self Pay Patient:
The fee for uninsured, self-pay patients will be equal to 100% prevailing Medicare Allowable. Payment is due at time-of-service. A payment plan (promissory note) may be considered upon request and with approval of BOM/Admin.

​Payment Plans:
Payment plans (promissory notes) are offered to patients and will be granted upon request and approval from BOM and /or Admin. Payment plans are not to exceed three (3) consecutive months starting from Date-of-Surgery. Any non compliance over 90 days will result in account being turned over to collection agency.
Collection Process:
Prior to your scheduled procedure, we will contact you with the results of the verification of their insurance benefits to advice you of your insurance deductible, and co-payment amount that will be due from you prior to your surgery.

​We expect the amount estimate due to be paid on the day of your surgery when you register at our admission desk. Accounts that have been deemed over 90- 120 days will be considered for Collection Agency submission. Your insurance carrier will determine your final patient responsibility.  You will be required to pay accordingly. Failure to pay may result in your account being turned over to our collection agency.
